Order Form
If you wish to order a 13x19 print, the following information is requested in order to begin the production process.
Prior to final printing, you will receive a digital preview via web link to ensure your satisfaction and final approval.
1. Rank/Rate and Full name (First MI Last)
2. "Call sign"
3. Squadron assigned and tail call letters
4. Detachment number and name (if applicable) (email image of det patch if desired in final artwork)
5. Dates assigned to squadron, include report and detach dates (day-month-year)
6. Aircraft Type/Model/Series (TMS: i.e. Sikorsky SH-60B Seahawk, Sikorsky MH-60H Romeo, Bell TH-57C JetRanger)
(* if special paint scheme requested, please provide photo of aircraft, sono cover, special markings, etc)
7. Aircraft side number and bureau number (BUNO)
8. Aircraft equipment (i.e. FLIR, Hellfire, Chaff/flare dispenser, etc)
9. Full mailing address for shipment
